Program
Capitation program providing intensive, wrap-around services to the city's most severe and persistently mentally ill. Members voluntarily choose to leave a state or community hospital setting and join this community-based program.
All Ages
Adulthood (22+)
English
24/7 assistance, medication monitering, housing, supported employment, substance recovery, crisis management, education, social and recreational activities.
Baltimore City Resident, Primary Diagnosis of Major Mental Disorder in DSMV, Able to provide consent or have guardian who will do so on client's behalf, Must meet one or more of the following criteria: -Stayed in a state psychiatric hospital for 6 months or more -Was an inpatient at a psychiatric unit at least 4 times in the last 2 years -Was admitted to an emergency room for psychiatric reasons at least 7 times in the last year
