A faxed copy of the license/certification being used to support the SAP practice under Federal DOT regulations MUST accompany or follow this order. Licensed physician (Doctor of Medicine or Osteopathy) Licensed or certified social worker Licensed or certified psychologist Licensed or certified employee assistance professional Alcohol and drug abuse counselor national level certification through NAADAC Certification Commission or ICRC Alcohol and Other Drug Abuse (state level certification does NOT meet the DOT requirements.)
