Requesting an IEP or 504 Evaluation

Public schools are required to provide help to children with certain challenges learn. If you think your child needs additional help at school, you should request an evaluation. This evaluation will assess if he/she qualifies for special education services or accommodations. You can request an evaluation at any time. Before requesting an evaluation, talk to your children's teachers. It is best to submit a written request to the principal of your child's school. Keep a copy of your request (including the date) for your records. Below is an example of a sample request letter. There are also tips for writing the letter (tips are in italics).

Dear Mr. Warner,
(the letter should be addressed to the principal of your child's school)
I am writing to request that my son, Joseph Mitchell Black (date of birth: 6/8/01) be evaluated for special education services or accommodations granted under Section 504 and the Individuals with Disabilities act. I am concerned that Joseph is struggling in school and he may need additional help and support to be successful.
(Be sure to include your child's full name and date of birth)
For the past six years, since Joseph was in Kindergarten, teachers have told me that he does not focus on his work, frequently gets out of his seat during class, disrupts other students, and does not follow directions. At home Joseph struggles completing his homework and does not seem to understand class materials. Teachers also report that Joseph is not reading at the same level of students his same age. I have also witnessed Joseph's struggles with reading at home. Joseph frequently receives poor grades, and I feel his grades are not matching his potential.
(In the letter, say specific reasons why you feel your child may need special help at school. Say how long ago the problems began.)
I also want to let you know that Joseph has recently been diagnosed with Attention Deficit Hyperactivity Disorder (ADHD) by his pediatrician, Dr. Rose. Dr. Rose feels that Joseph's ADHD may be interfering with his ability to do well in school. Dr. Rose also thinks Joseph may have a Reading Disability. I would be happy to provide you with the report Dr. Rose has written.
(Your child does not need to have a diagnosis for you to request an evaluation from the school. If your child does not have a diagnosis, skip this part. If your child does have a diagnosis, be sure to include what the diagnosis is and who you received the diagnosis from. Be willing to share any reports generated about the diagnosis.)
I understand that this evaluation will be provided free of charge. I would like to be involved in the evaluation in whatever way possible. I would like to meet with the person doing the evaluation beforehand and obtain more information about the process. I understand that I must provide written consent for the evaluation and I am happy to do so.
