Program
Contracts with the program contractors, managed care organizations providing medical, long-term care services (nursing home and home and community-based services), behavioral health services and case management. Individuals can apply for services at any one of the 14 offices located throughout the state. Anyone requesting services should contact the nearest field office. There are no application fees, but if determined eligible the client may be responsible for a monthly share of the cost payment.
