Description

The program provides home-based nursing services for infants and children to age 3 years who are at risk for preventable illness, disability or developmental delays due to medical, social or environmental problems. Nursing services include a health and developmental assessment, anticipatory guidance, health education and counseling based on the plan of care for the identified problems. These services are provided in collaboration with medical services by the primary provider. Infants are referred from hospital neonatal intensive care units, private medical physicians, social workers and other home health agencies. Coordination with other disciplines and organizations' resources is an important component of the total services.

Services Provided

Makes home visits when an infant is born with high risk medical, social or environmental problems. Follow-up parenting counseling is provided where necessary. Follow-up parenting education, counseling, and case management are provided. Developmental evaluations are completed.
